• Medical Release Form It is highly recommended that teams download and complete this Medical Release/Consent Form. This should be mailed in or turned in at check-in. This will be given to the trainers/doctors who are on-site for the tournament. The consent form is needed by the trainers in order to provide pre- and post-game therapy/care and chiropractic care. Players who are 18 years old can sign for themselves; minors must have a parent/guardian's signature. If this form is not completed in advance, the players/parents will be asked to sign at the tournament prior to service, and underage players without parent consent will not be eligible for non-emergency care.
